Service Specification - Unity Supported Living Services Ltd

Unity Supported Living Services Ltd provides housing-related support only and does not provide personal or regulated care under the Health and Social Care Act 2008. The service promotes independence and tenancy sustainment for adults living within the community.

Client Group

Adults aged 18+ who are able to manage personal care independently but require low to medium housing-related support to maintain accommodation and develop independent living skills.

Eligibility Criteria

Individuals must be suitable for shared or self-contained accommodation, able to engage with support, and not require 24-hour personal care or nursing provision.

Referral Process

Referrals are accepted from local authorities and professionals. An assessment is completed prior to placement and suitability confirmed before move-in.

Support Provided

Support includes tenancy sustainment, budgeting guidance, correspondence management, appointment attendance, independent living skills and community integration.

Support Not Provided

The service does not provide personal care, medication administration, clinical care, or regulated activities. External registered care providers are arranged where required.

Staffing & Hours

Support is delivered through scheduled sessions. Staff are not present to provide continuous care and emergency services are contacted where necessary.

Safeguarding

All safeguarding concerns are reported to relevant authorities in line with local safeguarding procedures.

Outcomes & Move-On

The aim is progression to greater independence and eventual move-on to independent accommodation where appropriate.

Housing Model

Residents hold occupancy agreements or tenancies. Housing and support are separate services.
